The DS-11 is the U.S. Department of State's application form for a new U.S. passport. It's the form nearly every first-time applicant needs — whether you're applying for the first time as an adult, applying on behalf of a minor, or replacing a passport that's no longer valid for renewal by mail.
You must file a DS-11 (rather than the simpler DS-82 renewal form) if any of the following apply: this is your first U.S. passport, your previous passport was issued before you turned 16, your most recent passport was issued more than 15 years ago, your previous passport has been lost, stolen, or significantly damaged, or your name has changed since your last passport was issued and you can't legally document the name change to the State Department's satisfaction.
Unlike a renewal, the DS-11 cannot be submitted entirely by mail — you'll need to appear in person at an authorized passport acceptance facility (many post offices, clerks of court, and public libraries offer this service) or a passport agency, with your completed form, evidence of citizenship, a valid photo ID, and a passport photo.
